NJACP, in collaboration with the other provider associations, developed a form to be completed by providers when they have questions or concerns following a meeting with representatives from DHS/DDD. For example, a problem could arise from an inspection where the provider is cited for a policy that has not yet been shared with the provider community or there may be a problem with a case manager‘s position on provider requirements, such as a case manager not accepting information electronically. In these cases, the form should also be completed. Collecting this information is extremely helpful with bringing issues to the attention of DHS/DDD.
